Clinical Case Manager needed to provide outreach, engagement and intensive case management services to adults who are homeless and addicted.

Experience linking people to housing, health care, entitlements and treatment preferred. Understanding of harm reduction along with a demonstrated passion for serving chronically homeless individuals with co-occurring disorders required.
